Please read these Terms of Service carefully before using the Dyslexia Evaluations website or services. By accessing our website or engaging our services, you agree to be bound by these terms.
1. About Our Services
Dyslexia Evaluations LLC provides dyslexia screening and psychoeducational evaluation services in Madison, Wisconsin. Our services include:
- A free online risk indicator questionnaire (CLDQ-R) to assess whether a child may benefit from further screening.
- Free in-person dyslexia screening using the WIAT-4 Dyslexia Index, administered by a qualified practitioner.
- Paid psychoeducational evaluations conducted by or under the supervision of a licensed practitioner.
These services are provided to parents and guardians of children who may be experiencing reading or language-based learning difficulties.
2. Not a Substitute for Medical or Educational Advice
The information provided on this website and through our screeners and evaluations is for educational and informational purposes. Our screening and evaluation services are intended to identify characteristics of dyslexia and provide recommendations — they are not a substitute for advice from your child's physician, licensed educational professional, or other qualified specialist.
A positive screening result does not constitute a formal diagnosis. A comprehensive evaluation report provides diagnostic impressions and recommendations, but educational placement and clinical decisions remain the responsibility of qualified professionals working with your family.
3. Eligibility
Our services are available to parents and guardians of children in kindergarten through 12th grade. By using our services, you represent that you are the parent or legal guardian of the child being screened or evaluated, and that you have authority to provide consent on their behalf.
4. Appointments and Cancellations
Free screener appointments are offered on a limited availability basis. If you are unable to attend a scheduled appointment, please notify us as soon as possible by emailing info@dyslexiaevals.com so we can offer the spot to another family on our waitlist.
Repeat no-shows may result in being removed from our scheduling list.
5. Fees and Payment
The risk indicator questionnaire and in-person WIAT-4 screener are provided at no charge. Psychoeducational evaluation fees are as follows:
- Standardized Dyslexia Screening
- Single Academic Assessment
- Comprehensive Diagnostic Evaluation
Payment is due prior to the completion of testing, as described in your signed Permission to Assess form. Payment is handled directly between you and Dyslexia Evaluations LLC and is not processed through this website.
Fees are non-refundable once testing has been initiated, except in cases where the evaluation cannot be completed due to circumstances on our end.
6. Confidentiality and Records
All assessment results, evaluation reports, and related clinical records are kept confidential. Records will only be shared with designated school personnel if you provide explicit written consent. Please refer to our Privacy Policy for full details on how we handle your information.
7. Parental Consent
All assessment services require a signed Permission to Assess form from the child's parent or legal guardian. By signing this form, you acknowledge the purpose of the assessment, the applicable fee, and the nature of the services being provided. No assessment will proceed without a completed and signed consent form.
8. Limitations of Screening
Our free screener is designed to identify whether a child shows characteristics that may be consistent with dyslexia and whether a full evaluation is recommended. A screener is not a diagnostic evaluation. A screening result — whether flagged or not — does not rule out or confirm the presence of dyslexia or any other learning disability. Families with ongoing concerns are encouraged to pursue a comprehensive evaluation.
9. Intellectual Property
All content on this website, including text, layout, design, and graphics, is the property of Dyslexia Evaluations LLC. You may not copy, reproduce, or distribute any content from this site without our prior written permission.
10. Limitation of Liability
Dyslexia Evaluations LLC provides services and information in good faith. To the fullest extent permitted by law, we are not liable for any indirect, incidental, or consequential damages arising from your use of this website or our services. Our total liability in connection with any claim related to our services shall not exceed the fees paid by you for the specific service giving rise to the claim.
11. Governing Law
These Terms of Service are governed by the laws of the State of Wisconsin. Any disputes arising under these terms shall be resolved in the courts of Dane County, Wisconsin.
12. Changes to These Terms
We reserve the right to update these Terms of Service at any time. Changes will be posted on this page with an updated date. Continued use of our website or services after changes are posted constitutes your acceptance of the updated terms.
