Terms of Service
Please read these Terms of Service carefully before using the Dyslexia Evaluations website or services. By accessing our website or engaging our services, you agree to be bound by these terms.
1. About Our Services
Dyslexia Evaluations LLC provides dyslexia screening and educational evaluation services to families across Wisconsin, delivered live over Zoom from our base in Middleton, Wisconsin. Our services include:
- A free online risk indicator questionnaire (CLDQ-R) to assess whether a child may benefit from further screening.
- A free dyslexia screening using the Tests of Dyslexia Screener (TOD-S), given live over Zoom by a licensed reading specialist, either in a small group session (free) or in a private one on one session ($99), whichever the family chooses when booking. We occasionally offer in person group screenings at partner schools.
- A paid comprehensive educational evaluation, the Confident Reader Roadmap, administered over Zoom by a licensed practitioner.
These services are provided to parents and guardians of children who may be experiencing reading or language-based learning difficulties.
2. Not a Substitute for Medical or Educational Advice
The information provided on this website and through our screeners and evaluations is for educational and informational purposes. Our screening and evaluation services are intended to identify characteristics of dyslexia and provide recommendations. They are not a substitute for advice from your child's physician, licensed educational professional, or other qualified specialist.
A positive screening result does not constitute a formal diagnosis. A comprehensive evaluation report describes your child’s academic skill profile and provides recommendations, but educational placement and clinical decisions remain the responsibility of qualified professionals working with your family.
3. Eligibility
Our services are available to parents and guardians of children in kindergarten through age 21. By using our services, you represent that you are the parent or legal guardian of the child being screened or evaluated, and that you have authority to provide consent on their behalf.
4. Appointments and Cancellations
Free screener seats are limited. To reserve a free group screening seat we hold a card on file. The card is not charged for the screening itself. You may cancel or reschedule at no cost up to 24 hours before the session. A cancellation with less than 24 hours notice, or a missed session with no notice, carries a $49 fee.
Private one on one screenings ($99) are paid at booking and are refunded in full if you cancel or reschedule at least 24 hours before the session. Inside 24 hours, or for a missed session, the fee is forfeited.
Confident Reader Roadmap sessions are paid in full at scheduling. With at least 48 hours notice, your payment is held as a credit toward a future appointment. With less than 48 hours notice, or for a missed session with no notice, the payment is forfeited. In a documented emergency, contact us at info@dyslexiaevals.com and we will review the situation individually.
Repeat no-shows may result in being removed from our scheduling list.
5. Fees and Payment
Our current fees are:
- Risk indicator questionnaire (online): free
- Free group TOD-S screener over Zoom: free. A card is held on file to reserve the seat and is charged only under the cancellation terms in Section 4 ($49 for a cancellation inside 24 hours or a missed session).
- Private one on one TOD-S screener over Zoom: $99
- The Confident Reader Roadmap (comprehensive educational evaluation, two Zoom testing sessions, written parent and school reports, action plan, and results consultation): $3,000
From time to time we offer discount codes for the Confident Reader Roadmap. Any discount code applied at the time of booking is reflected on your invoice, and the amount shown on your invoice is the amount due. Fees are also stated in writing in the Payment Policy and Permission to Assess forms you sign before any paid service begins. There are no hidden charges, and you are never charged for testing your child does not need.
Payment for the Confident Reader Roadmap is collected in full at the time of scheduling, as described in your signed Payment Policy and Permission to Assess forms. Payment is handled directly between you and Dyslexia Evaluations LLC and is not processed through this website.
The Confident Reader Roadmap is backed by our Clarity Guarantee: complete the evaluation and attend the results consultation, and if you do not walk away with a clear understanding of what is going on with your child's reading and a specific written plan you can act on and hand to their school, we refund the fee in full. Apart from the Clarity Guarantee and the cancellation terms above, fees are not refundable once testing has begun, except where the evaluation cannot be completed due to circumstances on our end.
6. Confidentiality and Records
All assessment results, evaluation reports, and related clinical records are kept confidential. Records will only be shared with designated school personnel if you provide explicit written consent. Please refer to our Privacy Policy for full details on how we handle your information.
7. Text Message Program (SMS) Terms
Program name. Dyslexia Evaluations Reminders, operated by Dyslexia Evaluations LLC.
Program description. When you provide your mobile number and agree to text messages while booking, you will receive recurring SMS text messages from Dyslexia Evaluations LLC about your child’s screening or evaluation. These include appointment reminders, reminders to complete intake forms, and scheduling updates for the appointment you booked.
Message frequency. Message frequency varies. You will typically receive up to 6 messages per booked appointment.
Cost. Message and data rates may apply.
How to opt out. Reply STOP to any message to cancel. You will receive one confirmation message and no further messages after that. Reply START to resubscribe.
How to get help. Reply HELP to any message for help, or contact us at info@dyslexiaevals.com or (608) 438-3412.
Carrier liability. Carriers are not liable for any delayed or undelivered messages.
Consent is not a condition of service. You are not required to agree to text messages in order to book an appointment or receive any service from Dyslexia Evaluations LLC.
Privacy. Your mobile information and SMS consent are handled as described in our Privacy Policy. Mobile information and SMS consent are not shared with third parties or affiliates for marketing or promotional purposes.
8. Parental Consent
All assessment services require a signed Permission to Assess form from the child's parent or legal guardian. By signing this form, you acknowledge the purpose of the assessment, the applicable fee, and the nature of the services being provided. No assessment will proceed without a completed and signed consent form.
9. Limitations of Screening
Our free screener is designed to identify whether a child shows characteristics that may be consistent with dyslexia and whether a full evaluation is recommended. A screener is not a full evaluation. A screening result, whether flagged or not, does not rule out or confirm the presence of dyslexia or any other learning disability. Families with ongoing concerns are encouraged to pursue a comprehensive evaluation.
10. Intellectual Property
All content on this website, including text, layout, design, and graphics, is the property of Dyslexia Evaluations LLC. You may not copy, reproduce, or distribute any content from this site without our prior written permission.
11. Limitation of Liability
Dyslexia Evaluations LLC provides services and information in good faith. To the fullest extent permitted by law, we are not liable for any indirect, incidental, or consequential damages arising from your use of this website or our services. Our total liability in connection with any claim related to our services shall not exceed the fees paid by you for the specific service giving rise to the claim.
12. Governing Law
These Terms of Service are governed by the laws of the State of Wisconsin. Any disputes arising under these terms shall be resolved in the courts of Dane County, Wisconsin.
13. Changes to These Terms
We reserve the right to update these Terms of Service at any time. Changes will be posted on this page with an updated date. Continued use of our website or services after changes are posted constitutes your acceptance of the updated terms.
