Dyslexia Evaluations LLC ("we," "us," or "the Company") is a Wisconsin limited liability company that provides dyslexia screening and psychoeducational evaluation services. This Privacy Policy explains what information we collect through our website, forms, and communications, how we use it, and the choices you have. By using dyslexiaevals.com, submitting our screener or intake forms, or communicating with us, you agree to the practices described in this policy.

This policy covers our website, online screener, marketing, and general communications. Clinical and assessment records created in the course of evaluation services are additionally governed by the consent and intake documents you sign before services begin. Where those documents provide greater protection, they control.

1. Information We Collect

Information you provide to us

  • Contact information: your name, email address, mailing address, and mobile phone number.
  • Information about your child: name, age, grade level, school, and your responses to screening questionnaires, including questions about reading, language, and learning history.
  • Screening and assessment information: responses and results from the free screener and, if you become a client, evaluation scheduling and intake information.
  • Payment information: payments are processed by Stripe, our payment processor. We do not store your full credit card number on our systems.
  • Communications: emails, text messages, form submissions, and notes from phone or video calls with our team.

Information collected automatically

  • Usage data: pages visited, links clicked, approximate location derived from IP address, browser type, and device information.
  • Cookies and similar technologies: our website and forms use cookies and pixels, including the Meta (Facebook) Pixel, to measure advertising performance and to show you relevant ads. See Section 6.

2. How We Use Your Information

  • To deliver screener results and explain what they mean.
  • To schedule and provide screening and evaluation services.
  • To communicate with you by email, text message, and phone about your results, appointments, and next steps.
  • To send you educational resources and information about our services, if you have opted in to marketing communications.
  • To process payments and maintain business records.
  • To measure and improve our website, forms, and advertising.
  • To comply with legal obligations and protect our legal rights.

3. Text Messaging (SMS)

When you provide your mobile number and submit our screener or intake forms, you consent to receive text messages from us related to your results, scheduling, and next steps. Message frequency varies. Message and data rates may apply. You can opt out at any time by replying STOP to any message. Reply HELP for help.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except as necessary for our text messaging service providers to deliver the messages.

4. How We Share Information

We do not sell your personal information. We share information only as follows:

  • Service providers: companies that help us operate, such as our practice management platform, form and scheduling tools, payment processor, email and text messaging providers, video conferencing platform, and cloud storage. These providers may access information only to perform services for us.
  • Our team: employees and contractors who need the information to provide services, all of whom are bound by confidentiality obligations.
  • Legal requirements: when required by law, court order, or to protect the safety of a child or others, including obligations that apply to our staff as mandatory reporters under Wisconsin law.
  • Business transfers: if the Company is ever merged, acquired, or sells assets, information may transfer as part of that transaction, subject to this policy.

Testimonials and marketing: we use testimonials, photos, video, or other content that identifies you or your child in our marketing only with signed written consent from the parent or guardian. We may share stories and quotes in de-identified form, with names and identifying details removed.

5. Children's Privacy

Our website, forms, and services are directed to parents and legal guardians, not to children. We do not knowingly collect personal information directly from children under 13 online. Information about children is provided to us by their parents or legal guardians for the purpose of screening and evaluation services. If you believe a child has submitted personal information to us directly, contact us and we will delete it.

6. Cookies, Analytics, and Advertising

We use cookies and similar technologies to operate the site and measure marketing. This includes the Meta (Facebook) Pixel, which helps us measure ad performance and reach people who may benefit from our services. These tools may collect information about your visit, such as pages viewed and actions taken.

You can control cookies through your browser settings. You can adjust your advertising preferences on Meta platforms through your Facebook or Instagram ad settings. Opting out of advertising cookies does not affect your ability to use our services. Our site does not currently respond to browser Do Not Track signals.

7. Video Sessions and Recording

Screenings and evaluations are conducted by secure video conference using a healthcare-grade platform operated under a Business Associate Agreement. All screening and assessment sessions are video and audio recorded. We record sessions to verify the accuracy of administration and scoring. Recordings are kept confidential, are accessible only to our team for quality and scoring verification, are never shared with third parties, and are retained and destroyed in accordance with our record-keeping policies. Recording is a standard part of our assessment process, and consent to recording is included in the intake documents you sign before services begin.

8. How We Protect Information

We use administrative, technical, and physical safeguards designed to protect your information, including encrypted platforms for client records, access controls limiting who on our team can view information, and confidentiality agreements with all staff. Our practice management platform is operated under a signed Business Associate Agreement. No method of transmission or storage is completely secure, but we work to protect your information consistent with applicable law and professional standards.

9. Retention

We retain screening and evaluation records as required by Wisconsin law and professional record-keeping standards. Marketing and website data is retained only as long as needed for the purposes described in this policy.

10. Your Choices and Rights

  • Email: unsubscribe from marketing emails at any time using the link in any email.
  • Text: reply STOP to any text message to stop receiving texts.
  • Access and correction: you may request a copy of the information we hold about you or your child, or ask us to correct it, by contacting us.
  • Deletion: you may request deletion of your information. We may need to retain certain records where the law requires it.

11. Changes to This Policy

We may update this policy from time to time. The effective date at the top reflects the most recent version. Material changes will be posted on this page.
