SMS Consent Evidence

This page documents how Dyslexia Evaluations LLC collects consent to send SMS text messages. It exists so that carrier and Campaign Registry reviewers can verify our opt-in flow, which is collected on two public web forms: our online booking form and our free reading screener.

SMS program

  • Program name: Dyslexia Evaluations Reminders
  • Business: Dyslexia Evaluations LLC, Madison, Wisconsin
  • Message types: for parents who opt in on the booking form, appointment confirmations, appointment reminders, intake form requests, and scheduling of results meetings for an appointment they booked. For parents who opt in on the free reading screener, follow-up texts about their child's screener results, upcoming free screening dates, and our evaluation services.
  • Message frequency: varies. Booking opt-ins typically receive up to 6 messages per booked appointment. Screener opt-ins typically receive up to 4 messages per month.
  • Consent is never bought, sold, rented, or shared. Every number we message was given to us directly by that parent, who chose "Yes, text me" on our booking form or our free reading screener.

Opt-in 1: online booking form

Booking consent is collected on our public online booking form. The form is open to the public, requires no login or account, and is reachable from every page of dyslexiaevals.com and from the results page of our free reading screener.

After the visitor chooses a location, a service and a time, the booking form runs through three screens. The screenshots below were taken from the live form.

Step 2 of 3 – SMS consent

The visitor is shown the full SMS disclosure and must answer for themselves. Nothing is pre-selected. The field opens blank and the visitor picks either Yes, text me or No, email only. The field is optional, so a visitor can complete the booking without ever agreeing to receive text messages. Consent to receive text messages is not a condition of purchase and is not bundled with any other agreement.

Step 3 of 3 – Payment and terms

The visitor enters payment details and ticks a box, unchecked by default, reading “I have read and agree to the Terms of Service and Communication Consent.” That box is a separate agreement to our published terms. It is not the SMS opt-in. The SMS opt-in is the affirmative choice made on Step 2.

Consent language shown at the point of collection

This is the exact wording displayed directly above the consent field on Step 2, word for word:

Opt-in 2: free reading screener

Parents who take our free reading screener can also opt in. The screener is public, requires no login, and is embedded on our website.

Before the parent enters a mobile number, the screener asks "Can we text you about your child's results?" The answer choices are "Yes, text me" and "No, email only". The question is optional, nothing is pre-selected, and the parent sees their results whether or not they opt in. This is the exact wording shown under the question:

We only text parents who chose "Yes, text me". After opting in, the parent receives a confirmation text: "Dyslexia Evaluations LLC: You're opted in to texts about your child's appointments and screening. Message frequency varies. Message and data rates may apply. Reply HELP for help, STOP to opt out."

Privacy and data handling

Mobile phone numbers and SMS opt-in consent are used only to send the messages described above, to parents who opted in. We do not sell, rent, or share mobile phone numbers or SMS opt-in consent with any third party or affiliate for marketing or promotional purposes. Numbers are shared only with the messaging service provider that delivers our texts, and only so that those messages can be delivered.
