SMS Consent Evidence

This page documents how Dyslexia Evaluations LLC collects consent to send SMS text messages. It exists so that carrier and Campaign Registry reviewers can verify our opt-in flow, which is collected on the booking form hosted by our scheduling provider.

SMS program

  • Program name: Dyslexia Evaluations Reminders
  • Business: Dyslexia Evaluations LLC, Madison, Wisconsin
  • Message types: appointment confirmations, appointment reminders, intake form requests, and scheduling of results meetings, for an appointment the parent booked
  • Message frequency: varies, typically up to 6 messages per booked appointment
  • No marketing texts. We do not send promotional or marketing text messages.
  • Consent is never bought, sold, rented, or shared. Every number we message was given to us directly by that parent when they booked an appointment.

Where consent is collected

Consent is collected on our public online booking form. The form is open to the public, requires no login or account, and is reachable from every page of dyslexiaevals.com and from the results page of our free reading screener.

After the visitor chooses a location, a service and a time, the booking form runs through three screens. The screenshots below were taken from the live form.

Step 2 of 3 – SMS consent

The visitor is shown the full SMS disclosure and must answer for themselves. Nothing is pre-selected. The field opens blank and the visitor picks either Yes, text me or No, email only. The field is optional, so a visitor can complete the booking without ever agreeing to receive text messages. Consent to receive text messages is not a condition of purchase and is not bundled with any other agreement.

Step 3 of 3 – Payment and terms

The visitor enters payment details and ticks a box, unchecked by default, reading “I have read and agree to the Terms of Service and Communication Consent.” That box is a separate agreement to our published terms. It is not the SMS opt-in. The SMS opt-in is the affirmative choice made on Step 2.

Privacy and data handling

Mobile phone numbers and SMS opt-in consent are used only to send the appointment related messages described above. We do not sell, rent, or share mobile phone numbers or SMS opt-in consent with any third party or affiliate for marketing or promotional purposes. Numbers are shared only with the messaging service provider that delivers our texts, and only so that those messages can be delivered.

Opt-out and help

  • Reply STOP to any message to stop all text messages immediately.

  • Message frequency varies. Message and data rates may apply.
  • Carriers are not liable for delayed or undelivered messages.
